Community Health Improvement Plan
In 2022, the Bourbon County Health Department partnered with the University of Kentucky Public Health to conduct a Community Health Assessment (CHA) using the MAPP framework. Two forums and a community health survey were held between June and September to identify health concerns among residents. Key issues identified included mental health, substance use, and basic needs.
Following the CHA, a Community Health Improvement Plan (CHIP) was developed for 2022-2027, focusing on enhancing health outcomes in three areas:
- Housing
- Mental Health
- Alcohol, Tobacco, and Other Drugs
The document presented here outlines the goals and actions based on the findings from the community health assessment. Its purpose is to facilitate monitoring and community-driven interventions aimed at enhancing health outcomes related to each key area.
